The
name on the application should match the full name on your
government-issued photo ID. The name on the application will be used
to generate the test site scheduling permit which will be sent in the
mail. The name on the permit must match the government-issued photo ID
you will provide to gain admittance on the exam day.
Complete exam
eligibility documentation must be submitted at the time of
application. Please do not send original copies of eligibility
documents.
Requests for special
accommodations due to a disability must be submitted at time of
application.
The
membership discount is only available to current members of the Heart
Rhythm Society. You must be a member of HRS at the time of
application submission to receive the reduced rate.
Payment must be submitted with application. VISA, MasterCard and
American Express are accepted. Checks must be drawn in U.S. dollars from
a U.S. bank.
Cancellation & Refund Policy will be strictly enforced.
Applications are reviewed in the order they are received and take 3-4
weeks to process. You will be notified within 2-3 business days upon
receipt of your application. Upon approval, a confirmation letter and
receipt will be sent to your preferred e-mail address.
Applications must be received by the final application deadline
date in order to be accepted.
Candidates are responsible for informing IBHRE of any changes in address
or contact information. Scheduling permits and official score
reports will be distributed by the U.S. Postal Service. All other
correspondences will be sent via e-mail.
